This Notice of Privacy Practices describes how Shannon Schureman, LMHC of Flare Mental Health may use and disclose your protected health information (PHI), and how you can access this information. Please review it carefully.
Your privacy is important. As a mental health provider, I am committed to protecting the confidentiality of your health information and following applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).
Understanding Your Health Information
Protected health information (PHI) includes information about your mental health care, treatment, diagnosis, payment for services, and other health information that identifies you.
I create a record of the care and services you receive. This record allows me to provide quality care and meet legal and professional requirements.
How I May Use and Share Your Health Information
I may use or disclose your PHI for the following purposes:
Treatment
I may use and disclose information to provide, coordinate, or manage your mental health care. This may include consultation with other healthcare professionals involved in your care when appropriate and permitted.
Payment
I may use and disclose information needed to obtain payment for services. This may include submitting claims, verifying insurance benefits, and communicating with insurance companies or billing service providers.
Healthcare Operations
I may use and disclose information for practice operations, quality improvement, administrative activities, and other activities necessary to operate my practice.
Other Uses and Disclosures Allowed or Required by Law
I may disclose health information when required or permitted by law, including situations involving:
Preventing serious harm to yourself or others
Suspected abuse or neglect as required by law
Legal proceedings or court orders
Public health or safety requirements

Third-Party Services
Flare Mental Health may use third-party services to support administrative functions, including scheduling, billing, and insurance-related services. These services may include Headway.
When applicable, these service providers are expected to protect health information and use it only for permitted purposes.

You have rights regarding your protected health information, including:
Right to Access Your Records
You may request access to your health records and certain information maintained by my practice.
Right to Request Corrections
You may request corrections or updates to your health information if you believe information is incorrect or incomplete.
Right to Request Restrictions
You may request limits on certain uses or disclosures of your health information, although I am not required to agree to every request.
Right to Request Confidential Communications
You may request that I communicate with you in a specific way or at a specific location.
Right to Receive a Copy of This Notice
You may request a paper or electronic copy of this Notice of Privacy Practices.
Your Choices
You may have choices regarding how your health information is shared in certain circumstances. I will discuss these choices with you when required.
Changes to This Notice
I reserve the right to update this Notice of Privacy Practices. Any changes will apply to information already maintained by my practice as well as information created or received after the effective date of the updated notice.

Acknowledgment of Receipt
Clients receiving services from Flare Mental Health will be asked to acknowledge receipt of this Notice of Privacy Practices as part of the intake process.
